Lincoln Community Health Center provides ambulatory health and emergency services to residents of Durham County and the surrounding areas.

The Center is open the following hours:

8:00 a.m. - 5:00 p.m.

Tuesday

8:00 a.m. - 8:00 p.m.

Wednesday

8:00 a.m. - 5:00 p.m.

Thursday

8:00 a.m. - 5:00 p.m.

Friday

8:00 a.m. - 5:00 p.m.

Saturday

8:00 a.m. - 12:30 p.m.

There are providers available at all times. If you need to reach a doctor when the Center is not open, dial 956-4000. You will be connected with MEDCOM, our answering service. MEDCOM will take your name and number and one of our providers will call you.

The Center has four vans and drivers available to transport its patients, based upon financial and/or medical need. Transportation requests for registered Center patients may be made by dialing 956-4036 between 8:00 a.m. and 5:00 p.m Monday through Friday. These reservations should be made at least 24 hours in advance. A Wheelchair Van is available for disabled patients as long as there is adequate access to the home.

Fees for services are based on a sliding scale according to Federal poverty guidelines. Payment status is based on the number of people in family and total family income.

The direct line for prescription refills is 956-4056.The Pharmacy does not fill prescriptions from physicians or agencies outside the Center.

Any unresolved patient safety or quality of care concerns may be reported to the Joint Commission on Accreditation for Healthcare Organization.
