Lincoln Community Health Center provides ambulatory health and emergency services to residents of Durham County and the surrounding areas.

There are providers available at all times. If you need to reach a doctor when the Center is not open, dial 956-4000. You will be connected with MEDCOM, our answering service. MEDCOM will take your name and number and one of our providers will call you.

The Center has four vans and drivers available to transport patients, based upon financial and/or medical need. Transportation requests for registered patients may be made by dialing 956-4036 between 8:00 a.m. and 5:00 p.m Monday through Friday. Reservations should be made at least 24 hours in advance. A Wheelchair Van is available for disabled patients as long as there is adequate access to the home.

Fees for services are based on a sliding scale according to Federal poverty guidelines. Eligibility for sliding fee scale discounts is based on the number of people in family and total family income. However patients are served regardless of ability to pay.

The direct line for prescription refills is 956-4056.The Pharmacy does not fill prescriptions from physicians or agencies outside the Center.
