Diabetes Self Management
(1) I will take my medications every day as prescirbed by my
doctor.
(2) I will follow my diabetic, low fat diet to reduce my blood
sugar and cholestrol.
(3) I will try to obtain my idea body weight. I will lose___
pounds by my next office visit. (A 10% weight loss goal in the next six months is a reasonable
target.)
(4) I will take my blood sugars at home and will call if the
results are consistently below 60 or above 200.
(5) I will check my feet daily. If I notice a sore or an
irritation, I will seek medical attention.
(6) I will walk 30 minutes most days of the week. If I notice
chest pain, shortness of breath, or chest tightness, I will seek medical attention.
(7) I will stop smoking. Ask you provider about Lincoln's
smoking cessation program.
(8) I will visit the eye specialist every year or as
indicicated.
(9) I will take a baby aspirin or an enteric aspirin every
day.
(10) I will work hard to keep my HgA1c (3 month blood
sugar test) below 7, which is an average blood sugar level of less than 150.
(11) You can choose your own self-management goal too.
