Diabetes Self Management


(1)     I will take my medications every day as prescirbed by my doctor.

(2)     I will follow my diabetic, low fat diet to reduce my blood sugar and cholestrol.

(3)     I will try to obtain my idea body weight. I will lose___ pounds by my next office visit. (A 10% weight loss goal in the next six months is a reasonable target.)

(4)     I will take my blood sugars at home and will call if the results are consistently below 60 or above 200.

(5)     I will check my feet daily. If I notice a sore or an irritation, I will seek medical attention.

(6)     I will walk 30 minutes most days of the week. If I notice chest pain, shortness of breath, or chest tightness, I will seek medical attention.

(7)     I will stop smoking. Ask you provider about Lincoln's smoking cessation program.

(8)     I will visit the eye specialist every year or as indicicated.

(9)     I will take a baby aspirin or an enteric aspirin every day.

(10)     I will work hard to keep my HgA1c (3 month blood sugar test) below 7, which is an average blood sugar level of less than 150.
