Health Care for the Homeless Clinic

GOAL
Recognizing that homelessness and health are closely linked, our goal is to improve the overall wellbeing of homeless individuals through primary care and case management services offered on site and in collaboration with other service providers in the Durham area.

WHO WE SERVE

Lincoln’s Health Care for the Homeless Clinic welcomes men, women, and children (over the age of 6 months) who are living in emergency shelters, transitional housing, domestic violence shelters, on the streets, in cars, in the woods, or doubled up in unstable housing. We will provide services for up to one year after the individual has obtained stable housing, and will assist with transition to another provider at that time if necessary.

SERVICES

-Management of chronic health conditions such as diabetes, asthma, and heart disease.

-Hospital follow-up care

-Walk-in acute or urgent care

-Mental health and substance abuse screenings and referrals

-Routine Gynecologic care

-STD testing

-Access to medications, lab work, and diagnostic studies

-Referrals to specialty and dental care

-Translation and transportation

Please note that we operate on an appointment schedule but will make every attempt to see Walk-in patients when possible. Walk-in patients are seen on a first-come, first-serve basis.

HOSPITAL FOLLOW-UPS

We welcome new and established homeless patients for follow-up appointments following hospital discharge. However, we are only available to see patients 20 hours per week, therefore scheduling can be limited. Please call as soon as you know a patient has a discharge need. Every attempt is made to see Hospital follow ups within 3- 5 days.

To schedule a follow-up appointment please call (919-808-5640). Leave a message if needed as the clinic is closed during the morning hours.

Please consider prescribing medications from our pharmacy formulary in order to ease the transition of care (Drug Formulary).

HOURS

Afternoons: Monday – Friday, 1:00pm – 5:00pm

Please note that because of staff meetings, we do not hold afternoon clinic the fourth Wednesday of each month.
